What is ICSI?
Intracytoplasmic Sperm Injection (ICSI) is an advanced IVF lab technique in which a single, healthy sperm is injected directly into a mature egg to help it fertilise. It is performed as part of an IVF cycle, under the same embryology lab conditions, and the resulting embryo is then cultured and transferred in the usual way.
Who needs ICSI?
- Low sperm count, poor motility or abnormal sperm shape
- Previous IVF cycles with low or failed fertilisation
- Sperm retrieved surgically (testicular or epididymal extraction)
- Frozen or donor sperm available in limited quantity
- Use of previously frozen eggs
How ICSI differs from conventional IVF
In conventional IVF, sperm and eggs are placed together in a dish and fertilisation happens naturally. In ICSI, the embryologist selects a single sperm under high magnification and injects it directly into the egg using a fine glass needle. This removes the need for the sperm to penetrate the egg on its own, which is why ICSI helps specifically in male-factor infertility.
Step-by-step: how an ICSI cycle works
- Ovarian stimulation: injections grow multiple follicles, tracked by ultrasound.
- Egg retrieval: mature eggs are collected under light anaesthesia.
- Sperm preparation: a fresh or frozen sample is processed to select healthy sperm.
- Injection: one selected sperm is injected into each mature egg in the lab.
- Embryo culture and transfer: fertilised embryos are grown for a few days before transfer, as with standard IVF.
Success rate and what affects it
ICSI significantly improves fertilisation rates in male-factor infertility, but overall pregnancy success still depends on egg quality, the woman's age, uterine health and embryo development — the same factors that affect any IVF cycle. ICSI is a fertilisation technique, not a guarantee of pregnancy, and your doctor will explain realistic expectations based on your specific reports.
Is ICSI the right choice for you?
ICSI is not recommended for every couple — specifically when sperm-related factors are identified on semen analysis, or when a previous IVF cycle had poor fertilisation. A proper evaluation — semen analysis, hormonal tests and ultrasound — helps decide whether standard IVF or ICSI is more suitable for you.
