Understanding infertility
Infertility means not achieving pregnancy after a year of regular unprotected intercourse. It is common, affects both men and women, and is very often treatable. The cause may lie with the woman, the man, both partners or remain unexplained after tests. A careful evaluation is the foundation for choosing the right treatment.
Common causes in women
- Ovulation problems, including PCOS and thyroid disorders
- Blocked or damaged fallopian tubes, often after infection
- Endometriosis
- Uterine fibroids, polyps or adhesions
- Reduced ovarian reserve, especially with advancing age
Common causes in men
- Low sperm count, poor motility or abnormal shape
- Blockage that prevents sperm from reaching the semen
- Varicocele (enlarged veins around the testis)
- Hormonal problems, infections and lifestyle factors such as smoking and heat exposure
How infertility is diagnosed
Diagnosis begins with a detailed history of both partners, followed by examination and targeted tests: hormone levels, pelvic ultrasound, semen analysis and, if required, a tubal test or other investigation. Results are explained together so you understand the reason and the choices. See fertility testing and diagnosis.
Treatment: from simple to advanced
- Lifestyle and medical treatment for weight, thyroid, sugar, infections and cycle regulation.
- Ovulation induction with monitoring by ultrasound.
- Surgical correction of problems such as cysts, fibroids, polyps or adhesions, often by keyhole surgery.
- IUI when tubes are open and sperm quality is adequate.
- IVF and ICSI for tubal factor, severe male factor, endometriosis, unexplained infertility or after failed IUI.
- Donor or surgical sperm retrieval options where needed, within the law (see surgical sperm retrieval).
Lifestyle steps that help
Maintaining a healthy weight, regular exercise, balanced meals, stopping tobacco and limiting alcohol improve both egg and sperm health. Manage stress and sleep, take folic acid before conception, and avoid unprescribed herbal or hormonal products. These steps work alongside medical treatment; they do not replace it.
